works, emails are forward with and without copy. but in the 
postfixadmin interface the change are not present, only look DB table.
Well, if Postfixadmin doesn't display the values that are stored in 
the database, how is this is the fault of Forwards?

works! but still have error, the header patch failed. and in the
postfixadmin the change is not reflected.
Ignore the header. What do you mean with the last sentence? Does it 
work or not?
works, emails are forward with and without copy. but in the 
postfixadmin interface the change are not present, only look DB table.

Sorry, took a while to reply.

Have applied the patch. Note that your patch did not apply cleanly. 
One hunk failed. I went through customsql.php and manually added the 
bit that was not successfully applied.

I noticed that your patch assumes that the pre-existing version of 
customsql.php file should be version 1.21 :
* $Horde: forwards/lib/Driver/customsql.php,v 1.20 2009/04/27 
08:10:02 jan Exp $
However, the version distributed with forwards h3-3.2 is actually 
somewhat older :
* $Horde: forwards/lib/Driver/customsql.php,v 1.13.2.7 2009-08-18 
12:54:50 jan Exp $
I am not sure if this in any way relates to the problem in question.

Anyway, I finally do get somewhat more meaningful info in horde's log files :

Dec 17 01:42:25 HORDE [error] [forwards] DB Error: unknown error:   
[nativecode=1065 ** Query was empty] [pid 7820 on line 261 of 
"/www/horde/forwards/lib/Driver/customsql.php"]

I am not sure if this means that the query string was empty or the 
result of the query returned as empty.

I can confirm the error. Am also using the PostfixAdmin plugin with 
forwards 3,2 on CentOS 5.4. When the error message appears, horde's 
log file does not log anything, so it is impossible to troubleshoot 
further.

Note that in my case, I have also have vacation 3.2 installed and that 
works fine.

When configure module for postfixadmin this dont works. always say DB 
Error: unknown error
