I finally sat down and found a solution to this and got it working. What a pain. :)

Just a note that this only happens when you go to step 4 to assign the ticket on creation as well - with new tickets that stop at step 3, attachments work fine.

If specifying an attachment in step 3 of creating a new ticket, the attachment doesn't go anywhere.
