tokenizer driver?

- have genxref ignore known non-code files like .txt, .html, .css,
images, etc.
- try to recognize .inc as PHP
check out

and
http://cscope.sourceforge.net/
some includes not recognized by parser
static functions not recognized by parser
chained function calls not recognized by parser (see dbblue)
doesn't know about some builtins like trigger_error
integrate luxor, phpdoc, and chora
