Am also using the PostfixAdmin plugin with forwards 3,2 on CentOS 5.4. When the error message appears, horde's log file does not log anything, so it is impossible to troubleshoot further. Note that in my case, I have also have vacation 3.2 installed and that works fine. Have applied the patch. Note that your patch did not apply cleanly. One hunk failed. I went through customsql.php and manually added the bit that was not successfully applied.
