You need to add the "heading" entries to the $theme_icons array in themes/theme_name/info.php. Should be added to the themes howto in the wiki.

some icons is not theme-able. I mean ...you can not change the icon with your own icon set. Development icon in the sidebar /themes/horde/graphics/devel.png Office icon in the sidebar /themes/horde/graphics/office.png Organizing icon in the sidebar /themes/horde/graphics/organizing.png
